Piedmont Hospice encourages families to embrace a proactive approach to patient care and support via advanced care planning at an early stage to assure quality care – physically, emotionally, mentally and spiritually. The peace of mind provided by planning ahead will ease you and your loved one’s experience through the difficult times ahead.
Any individual can recommend a patient to Piedmont Hospice. The patient’s attending or primary care physician will be contacted for verification that the patient meets hospice admission criteria and for any other medical information.
Hospice admissions are coordinated by the Clinical Director. A nurse and/or social worker will meet with the patient and their family to provide information, answer questions and assist with admission into our hospice program.
