The sooner our Piedmont Hospice team is consulted and involved, the better for your loved one and you. The peace of mind provided by planning ahead will ease you and your loved one’s experience through the difficult times ahead.
Any individual can recommend a patient to Piedmont Hospice. The patient’s attending or primary care physician will be contacted for verification that the patient meets hospice admission criteria and for any other medical information.
Hospice admissions are coordinated by the Clinical Director. A nurse and/or social worker will meet with the patient and their family to provide information, answer questions and assist with admission into our hospice program.
